Proof of Custody and Residency (click here to print in English or Spanish)

This form is required for new students, students who have moved to a new address, or have had a change of guardianship. Two (2) proofs of residency must accompany this form. Instructions and the acceptable types of documentation are listed on the form. Each student will need their own form, which must be signed and dated. 

Health Services: Pre-Admission Health form (click here to print)

This form should be filled out for all Pre-K and Kindergarten students new to the district. This prints in English and Spanish.

This form is needed to allow the district to obtain records from your child's previous school.

All students are required to have proof of residency within School U-46 boundaries. If you are new to the district, recently moved, using a lease or affidavit, you will need to update your proof of residency at your school.  The following is required to prove residency:

1. Proof of Custody and Residency Form - English or Spanish
2. Two (2) Proofs of Residency as indicated in the form above
3. Affidavit of Resident Regarding Residency of Others
   This form is only needed if the parent/guardian is living with others, therefore do not have a lease, mortgage statement or real estate tax bill. This form MUST be updated annually. Please contact your school or the registration office for a copy.

Please note: Certain proofs of residency require annual updates, or when a document expires. If requested, the information will need to be turned into your child’s school before enrollment is considered complete. ALL Affidavits of Residency and leases must be updated annually.

The district reserves the right to investigate any claim of residency or request current documents and forms in order to determine residency.

U-46 2024-25 Fee Waiver Form (Click to print in English or Spanish)
It is reccommended that all families submit this form to see if they qualify for additional discounts. Currently, only optional fees (Driver's education and athletics fees) are being collected. 

Accepted forms of payment:

• Online payment: Debit/Credit Card and checking/savings account payments accepted online.  No processing fees charged! Payments may be made through the Infinite Campus Parent Portal.  Sign in to the Parent Portal and then click on "Fees" from the left side of the page.
• Payment in person: 
• Cash/Money Order - Accepted at all school levels
• Debit/Credit Card - Accepted at Middle and High Schools only. 

Optional fees must be paid at your child’s school.  See your school’s website for more information. Optional fees include: parking permit, customized yearbook, activity card fees.

Students entering Preschool, Kindergarten, 6th, 9th, and 12th grade are required to submit health records to the school.  Click here to see the requirements for your student. As an option, you may upload your child's physical and/or immunization records right to the Registration Application, or you may email them to the Health Services Department. To be fully registered, the following must be completed:

1. Online registration application submitted and approved
2. Approved Proof of Residency on file at each school
3. Up-to-Date Medical Information on file at each school
4. Required Fees Paid in Full  

Registration Checklist
Families of currently enrolled students can use their Campus Parent login to use our Registration Checklist. The Checklist shows which requirements are still missing for each student, or if their registration is complete. If you are a new family, once your enrollment is complete, you will be sent an email to set up your Campus Parent account.

From the Checklist you can also print a Proof of Enrollment letter for each student.

Current U-46 Parents who still need to pay fees for this school year can log in to the Infinite Campus Parent Portal.  Use the Fee selection on the left side of your screen. Fees are set up with a shopping cart to pay individually or in full. Between April 1st and May 31st, parents can save $20 per student by paying before May 31st. Fees will increase on June 1st for 2020-21. (PLEASE NOTE: for the 2022-23 school year, no general student instruction fees were due to assist families struggling from the effects of COVID 19. Pending school board approval, general instructional fees will also be waived for 2023-24. Please see our Student Fees section for more information.

2022-23 Kindergarten Registration

If you were unable to attend your school's Kindergarten Registration event held in February, there is still time to register. If you currently have students enrolled in U-46, and have not completed your annual registration, you can add your kindergarten-age child right to this year's application.
Please contact your school to set up an appointment.

To be age eligible to enter kindergarten next fall, children must be five years old on or before September 1, 2022. Parents should bring their child’s birth certificate and two documents showing proof of residency within U-46 boundaries.
