Welcome to School District U-46
New Family Registration GuideRegister NOW for the 2025-26 school year!
All students must be registered following these two steps:
Please read all the information before you start your online application.STEP 1 - Online Registration using the New Family Kiosk (see link and details below). All of your students will be registered on the same application.
STEP 2 - Contact your school to provide your application number, required documentation and forms or changes to household information.
For your child to participate in any of the Summer Connection Programs, they must be fully registered and enrolled for the 2025-26 school year. Please complete our 2-step process.

Are you new to the district for the 2024-25 school year?
If you are new to School District U-46 for the 2024-26 school year, our 2025-26 application will be good for both years.

Step 1
Online registration for new familiesNew Family Kiosk
- Click on the above link if you do not have students currently enrolled in School District U-46 or if your students previously attended prior to the current school year.
- You will be registering ALL of your children you wish to enroll in School District U-46 (PK-high school) at the same time, including any student that you would like to have screened for our preschool program*.
- When you enter the application site you will have the opportunity to start a new application, or return to one that you have already begun.
Please use one application. All of your children should be on the SAME application, no matter what U-46 school or program they will attend.You will then be provided with your application number as you enter the application. Please write this number down for future reference.
When you have completed all sections of the application, use the red submit button, and contact your school.

Step 2
When contacting your schoolBe prepared to provide the following:
- Student’s Birth Certificate
- Required forms (see the Printable Registration Forms section below)
- Proof of Residency from Category A + B as listed on the Proof of Custody and Residency form
- Enrollment will not be entered without acceptable residency documentation
- Physical and Immunization updates will be due prior to the first day of school
- Consent for Release of Records from previous school (1st grade and above only)
- Need assistance with registration
The school cannot refuse to admit or enroll a student because that student fails to present his/her student permanent or temporary records from a school attended previously.
What school will my child attend?
Students are assigned to schools based on their home address.

Printable Registration Forms
In addition to the Online Registration (OLR) application, there are other documents schools collect. Listed below are the most often requested forms that you can print and email to Registration@u-46.org.

Proof of Custody and Residency (click here to print in English or Spanish)
This form is required for all students and must be current and up-to-date. Two (2) proofs of residency must accompany this form. Instructions and the acceptable types of documentation are listed on the form. Each student will need their own form, which must be signed and dated.
Health Services
Tentative 2025-26 Health Requirements click here (English / Spanish)
Pre-Admission Health form (click here to print)
This form should be filled out for all Pre-K and Kindergarten students new to the district. This prints in English and Spanish.
Consent for Release of Records form (Click here to print in English or Spanish)
This form is needed to allow the district to obtain records from your child's previous school.

Home Language Survey
The Illinois State Board of Education (ISBE) requires all school districts to collect the Home Language Survey for all new students. This information is used to count the students whose families speak a language other than English at home. It also helps to identify the students who need to be assessed for English language proficiency.
While the Home Language survey is administered in English as part of our online registration, prior to starting your registration, if you would like to view the Home Language Survey in a language other than English, please visit the Multilingual Identification section of the ISBE website, and then go down to the Home Language Survey section.

Registration Fees
Note: Student Registration/Instructional Material Fees have been waived for the 2024-25 school year as financial relief to families.
Registration (instructional materials) fees have been approved for the 2025-26 school year.

- Online payment: Debit/Credit Card and checking/savings account payments accepted online. No processing fees charged! Payments may be made through the Infinite Campus Parent Portal. Sign in to the Parent Portal and then click on "Fees" from the left side of the page.
- Payment in person:
- Cash/Money Order - Accepted at all school levels
- Debit/Credit Card - Accepted at Middle and High Schools only.

Residency
All students are required to have proof of residency in the School U-46 boundaries. If you are new to the district, recently moved, using a lease or affidavit, you will need to update your proof of residency at your school. The following is required to prove residency:
- Proof of Custody and Residency Form - English or Spanish
- Two (2) Proofs of Residency as indicated in the form above
- Affidavit of Resident Regarding Residency of Others
This form is only needed if the parent/guardian is living with others, therefore do not have a lease, mortgage statement or real estate tax bill. This form MUST be updated annually. Contact your school or the registration office for a copy.

Android PhoneIf you are having difficulties with housing or cannot provide the required residency documentation, please contact Project Access at 847-888-5000 ext. 6768, 6769 or 6765.
Please note: Certain proofs of residency require annual updates, or when a document expires. If requested, the information will need to be turned into your child’s school before enrollment is considered complete. ALL Affidavits of Residency and leases must be updated annually.
The district reserves the right to investigate any claim of residency or request current documents and forms in order to determine residency.

Health Requirements

This form should be filled out for all Pre-K and Kindergarten students new to the district. This prints in English and Spanish.
Students entering Preschool, Kindergarten, 6th, 9th, and 12th grade are required to submit health records to the school.
As an option, you may upload your child's physical and/or immunization records right to the Registration Application, or you may email them to the Health Services Department. For questions, please email healthservices@u-46.org.

Early Childhood Information
All students should be added to the application, no matter what grade or school. Below is information regarding our Early Childhood program.
Kindergarten Registration
If you have not registered your kindergarten age student, please include them on your registration application.
To be age-eligible to enter kindergarten next fall, children must be five years old on or before September 1, 2024. Parents should bring their child’s birth certificate and two documents showing proof of residency within U-46 boundaries. No fees will be collected at this time.
Preschool
Preschool age children need to be screened in order to qualify for placement. Please call 847-888-5000 ext. 6094 to make a screening appointment. New or returning preschool students will not be scheduled into a class until all required paperwork is received. -
