Whether it's your teacher or your cousnelor, if you're aksing for a Letter of Reccomendation, it helps to give them a little extra information about yourself. This form is a great place to start!  Remember it also takes time for anyone to write a good Letter of Recommendation, so make sure you give whomever you're asking to write the letter plenty of time before the deadline!
